OPACT- Orange Primary Access Coordination Team

OPACT is a part of a free community program to link people without health insurance to the care they need.

The Orange Primary Access Coordination Team (OPACT) consists of a Registered Nurse, a Registered Dietician, and an Eligibility Specialist who work in the Orange County community to identify families without health insurance and:

• Help them with paperwork and conduct eligibility screenings for insurance programs such as Medicaid and Kid Care
• Conduct health screenings and risk assessments for chronic diseases
• Provide case management and health education
• Link people to a health care provider (medical home)

Our staff is multilingual and can provide services in English, Spanish and Creole.
