Commercial Systems - Existing

Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.

The following forms are required for a Commercial Existing Permit:

• DH4015-1 Construction Permit Application
• DH4015-4 Existing and Repair Certification
• Site Information Documentation Form
• DH4015-3 Soil Evaluation Form
• Survey to Amend Annual Operating Permit
